About Us Southern Comfort Home Care is a growing private duty home care agency dedicated to helping seniors and individuals remain safe and comfortable in their homes. We provide personalized non-medical and skilled support services while delivering exceptional customer service to our clients and families. We are seeking a compassionate, organized, and proactive RN Care Manager to oversee client care coordination, collaborate closely with the Alternate Director of Nursing, support caregivers, and ensure clients receive outstanding care. Position Summary The RN Care Manager serves as a key point of contact for clients, families, caregivers, and referral partners. This role is responsible for coordinating with the ADON regarding all aspects of care, conducting assessments, evaluations, supervisory visits, monitoring client satisfaction, and ensuring high-quality care delivery. The ideal candidate is clinically knowledgeable, highly organized, and thrives in a fast-paced environment while building strong relationships with families and staff. Responsibilities Develop and update individualized charts Monitor client progress and identify changes in condition Communicate with clients, families, physicians, hospitals, and referral sources as needed Admit and Evaluate Clients in their homes upon admission condition changes Provide caregiver training, guidance, and clinical support Address client concerns and resolve service issues quickly Maintain accurate documentation and ensure regulatory compliance Assist with client onboarding and service start-ups Support retention efforts by ensuring excellent client experiences Participate in on-call responsibilities Qualifications RN License in good standing Strong communication skills Excellent organizational and time management abilities Ability to work independently and make sound decisions Valid driver's license and reliable transportation required Preferred Qualifications Experience in private duty home care or home health Experience working with seniors and families Familiarity with scheduling or home care software is a plus Benefits Paid time off Health Insurance benefits 401K Growth opportunities within a rapidly growing company Why Join Our Team? Pay:
$70,000.00 - $85,000.00 per year
