The RN Supervisor will provide clinical leadership and oversight to ensure patients receive safe, high-quality, individualized care in the comfort of their own homes. The ideal candidate is a strong clinical leader who enjoys supporting both patients and staff, is comfortable working independently, and is committed to maintaining the highest standards of patient care and regulatory compliance. Key Responsibilities Provide clinical supervision and support to home health nurses, aides, and other clinical staff. Oversee the delivery of skilled nursing and home health services to patients in their homes. Review patient assessments, care plans, orders, and clinical documentation for accuracy and completeness. Conduct and/or oversee comprehensive patient assessments and reassessments (OASIS). Monitor patients' conditions and ensure appropriate interventions are implemented when changes occur. Communicate with physicians and other healthcare providers regarding patient needs, changes in condition, and plan of care. Coordinate patient services with therapy, social services, home health aides, physicians, and other members of the interdisciplinary team. Provide guidance and clinical support to field staff regarding patient care concerns and complex cases. Participate in staff orientation, training, competency evaluations, and ongoing education. Monitor staff performance and provide coaching and corrective guidance when necessary. Ensure timely completion and review of clinical documentation. Monitor compliance with agency policies, state and federal regulations, and applicable Medicare/Medicaid requirements. Participate in quality assurance and performance improvement activities. Assist with audits, chart reviews, incident investigations, and corrective action plans. Help ensure appropriate staffing and patient assignments. Participate in an on-call rotation, if applicable. Respond promptly and professionally to patient, family, physician, and staff concerns. Promote infection prevention and control practices. Maintain confidentiality and protect patient rights and privacy. Serve as a positive role model and clinical resource for the entire home health team. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of Iowa. Previous home health experience strongly preferred. Previous supervisory, management, or clinical leadership experience preferred. Knowledge of Medicare/Medicaid home health requirements preferred. Strong assessment, clinical judgment, and critical-thinking skills. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to work independently and effectively manage multiple priorities. Proficiency with electronic medical records and clinical documentation systems. Current CPR/BLS certification. Valid driver's license, reliable transportation, and appropriate auto insurance if required for field visits. Must successfully complete all required background checks and employment requirements.
