Job Description

Legacy Coordinator •
Objective:
The primary goal of the Legacy Coordinator is to act as a buffer for the building. Their duty is to respond to all walkie requests and see if they can resolve any issues on the floor. If they are unable to do so, they may need to ask for help from the nurse/administrator. They are an integral part of the administration team, ensuring that all the team members are trained, taken care of and completing their assigned tasks. This will ensure proper care for our residents. •
Duties and Responsibilities:
Delegated Supervision of Resident Cares Oversee the training of team members to provide care appropriately. Oversee how-to hands on training and observe trainers/trainees providing personal hygiene, peri-cleansing, dressing, and transfer assistance ensuring safety, efficiency, and thoroughness. Maintaining all nursing supplies and notifying resident care coordinator of needs. This includes but not limited to resident attends, gloves, wipes, cups, med cups, UA supplies, and oxygen tubing. Make daily rounds to ensure that staff are working in assigned areas and performing the duties of their jobs related to the Negotiated Service Agreement for each resident. (ADLs, assistive devices, meds/treatment, beds made, laundry, done, room tidy, etc. (per NSA). Hold "stand up" meeting with team members (AM, PM and Night Shift) to communicate new or ongoing issues related to resident care (appointments, admits, discharges, incidents, behaviors, new temporary care plans, alert charting etc…) Ensure that weights, vital signs, blood glucose checks, oxygen monitoring, skin observation sheets and other special resident care needs are completed, documented and communicated to the nurse, as needed. Notify nursing of any errors, incomplete documentations, or other issues. All concerns or deviations from expected levels of care will be communicated to the nursing daily, and as needed. Welcome new residents and check with them daily to ensure happiness during the first week in our building. Notify nursing or administration of any concerns. Communicate with floor team members related to specific resident needs (new and existing residents). Ensure all delegations, licenses, certificates, continuing education yearly (8 hrs), etc. of team members in your wing are current and up to date. Schedule delegations of new team members with nurse and receive prior approval of time and date. Make bi-monthly room rounds to ensure that soap, paper towels, trash cans, beds, toilets are clean, tall shelves & dressers are secured to wall, and any other issues are brought to nursing/administration attention. Perform assigned medication cart audits weekly and as needed depending on needs of facility as directed by nursing and administration . Complete incident reports and assist with investigations (as needed). Participate in meetings with residents, families, Ombudsman, survey agencies and other entities (as needed). Respond to emergency calls involving residents (provide first aid or initiate EMS) (as needed). •Medication Related Tasks Oversee and monitor proper medication passing techniques by observing team members procedures of hand washing, passing meds, documentation, etc. Oversee, train and observe infection control practices and Standard Precautions. Assist with medications as directed by nursing. Weekly med cart audits Checking for correct medication dates Cleaning carts Making sure cart are stocked Write list of meds running low and submit to nursing for re-ordering Finding med errors and submit them to nursing, assist in investigation as directed (but never completing coaching for med error, Must have nurse signature and education in coaching) Remove expired meds, medications of residents no longer in the building, etc. Ensure proper storage of medications in cart. Ensure a med kit is in the cart when audited (pulse oximeter, BP cuff, thermometer, stethoscope, etc). Ensure each med cart has its assigned pager daily and that it is functioning correctly. Ensure that all PRN medications are available. Notify administration when the sharps container is full or when a sharps container is needed. Oversee or prepare medication list for residents going out of the building. (You or Shift lead) Prepare medications (clean out med cart and ensure proper documentation) for residents being transferred or discharged from facility. Weekly narcotic count audits done at the time of medication cart audits. Communicate any concerns to nursing and Administrator, immediately. Weekly room rounds to ensure that residents do not have prescription or over the counter medications in their rooms, unless they have current signed orders for self- medication privileges. Ensure soap, paper towels, trash cans are in rooms to ensure caregivers can properly take care of residents and notify resident care coordinator of needs. Check in cycle meds and compare with MAR. Medication delivery must be checked in on Saturday mornings or delegated to shift lead if coordinator is unable to be in. Assist Nurse with destruction and returns of medications. •Team Member Supervision On-call 24/7 for staffing issues. (Refer nursing related calls to on call nurse). Complete monthly schedule for caregivers and medication assistants. Monitor and report attendance concerns for your wing of the building. Legacy Coordinator will be responsible for their wing's med carts for all shifts and the am and pm float as well as the laundry aid. Ideally both coordinators should communicate about the needs for their team members and work together in scheduling team members (requested days off, limitations of days worked, hours needed to provide for family, etc.) Overtime should be avoided All missed breaks, staying late or coming early need to be pre-approved/approved at time occurance. Follow the team member guidelines in the employee packet. Each coordinator will need to turn in any approvals for time-clock corrections in a timely manner. Update float list for respective wings of building. Monitor the staffing schedule to ensure adequate staff are available to meet the needs of the residents. Ensure that staff receive an adequate report on his/her assigned residents at the start of the shift. Act as a role model for non-licensed staff in the delivery of safe, competent and compassionate service. Monitor, train and ensure shift leads are doing their duties. Provide instruction for staff related to resident care needs and standards of practice. Provide direction and support to facilitate smooth delivery of nursing care on a daily basis. Ensure cares, ADLs, observations on MAR are accurate. Oversee and monitor fridge temp logs in respective wings of building. Assist nurse in completing necessary disciplinary actions (all med errors are to be completed by a nurse). Handle team member concerns/problems as they arise, daily. Direct team members to direct supervisor for resolution of issues (if no resolution obtained by direct supervisor, refer them to administration). Participate in the hiring and orientation of new nursing department team members. Participate in the evaluation of nursing department team members. Assist front desk when they are away (answering phones etc.) You are required to cover any uncovered shifts for your hall. You may also schedule yourself on a med cart if you would like to, but your duties will still need to be done for the day (as needed). •Record keeping Supervise documentation in ECP to ensure timely and complete entries. Report and document changes in resident condition to nursing/administration. Report and document incidents or behaviors according to facility policies. Complete charting of medications, treatments, and other assigned tasks in ECP in timely manner. Ensuring that reportable incidents are reported to the Administrator. Update behavior tracking and binders. Print new and file used Insulin Logs. Update Red File monthly (medication list and Post). Keep up to date on ECP follow up in google drive. Quarterly cognitive assessment for legacy residents. •Coordination of Services Communicate about medications as directed by nursing, which may include contacting the VA, pharmacies, families and residents, etc. Communicate with nursing and administrator concerning observations of resident care needs and staff performance. Communicate with administration about supplies and suppliers. Assist with admission, transfer and discharge coordination. Communicate with outside agencies (as needed). Participate in monthly Fall Committee. •Requirements Current Medication Tech certification in the State of Idaho, first aid/CPR certification, with at least 1 year of experience with at least one year caring for the elderly, preferably in Assisted Living or Long Term Care. Successful completion of all new hire background check requirements. Speak, read and write English. Effective oral and written communication skills. Skilled in recognizing physical, mental, and emotional needs of residents and team members. Ability to work closely with professional and non-professional team members. Knowledge of Federal, State, and local regulations. Able to maintain patience, poise, and dignity in emergency situations. Desire to work with the elderly Able to keep confidentiality of our team member & residents. Good judgment and ability to make educated independent decisions. Emotional stability, tact and understanding of residents' physical, psychological, social, and emotional needs. Able to prioritize multiple tasks and take/give instructions well (supervisory skills). Regular, predictable, and dependable attendance. Schedule will require at least one coordinator in the building 7 days/week and that each coordinator work at least five days a week. Both coordinators should be at the Wednesday administrative stand up from 8:45 am- 9:30 am. All other shifts can vary. Except for the rotating on call schedule. Each coordinator will be on call 2 weekends out of the month (Friday night - Sunday night). (The schedule will need to be approved by administrator and should be worked on together). Wages Salary, D.O.E •Other tasks not disclosed in this document may be added by nursing/administration (PRN).•
