A Clinical Manager RN in a home health company serves as the clinical leader responsible for overseeing patient care quality, clinical staff supervision, and regulatory compliance. The ideal candidate will be responsible for clinical oversight, staff management, quality assurance, and ensuring the smooth operation of our home health care services. Responsibilities Clinical Oversight Review and approve OASIS assessments for accuracy and compliance Oversee the development and implementation of individualized plans of care Ensure clinical documentation meets Medicare, Medicaid, and state regulatory standards Conduct case conferences and clinical case reviews with the interdisciplinary team Monitor patient outcomes and identify opportunities for improvement Staff Management Supervise, mentor, and evaluate field clinicians (RNs, LPNs, PTs, OTs, STs, HHAs) Conduct staff orientation, competency assessments, and ongoing education Manage clinician caseloads and visit scheduling in coordination with intake/scheduling staff Address performance concerns and conduct disciplinary actions when needed Conduct supervisory visits and skills competency evaluations per regulatory requirements Quality Assurance & Compliance Lead quality assurance/performance improvement (QAPI) initiatives Ensure agency compliance with CMS Conditions of Participation, state licensure, and accreditation standards Prepare for and participate in state and accreditation surveys Track and report clinical quality measures (HHCAHPS, OASIS outcomes, star ratings) Investigate and resolve incident reports, complaints, and adverse events Patient Care Coordination Serve as clinical point of escalation for complex patient cases Coordinate with physicians regarding orders, plan of care changes, and recertifications Ensure timely completion of start of care, resumption of care, and discharge processes Collaborate with intake staff to evaluate referral appropriateness and clinical eligibility Ensure appropriate utilization of services and resource management Required Skills and Qualifications Active, unrestricted Registered Nurse (RN) license in the state of Illinois Strong knowledge of Medicare Conditions of Participation and OASIS requirements Proficiency in healthcare management software and electronic health records (EMR) systems Excellent leadership, interpersonal, and written/verbal communication skills Superb critical-thinking, organizational, and analytical skills Ability to work with diverse age groups and populations Current CPR certification Preferred Skills and Qualifications COS-C (Certificate for OASIS Specialist - Clinical) or similar OASIS certification Experience in home health quality assurance/performance improvement (QAPI) Proficiency in more than one language
