Job Description

Options Home Care is seeking an experienced and compassionate Registered Nurse (RN) Care Manager to join our home care team. This is an excellent opportunity for an RN who enjoys working independently, building relationships with clients and caregivers, and using their clinical expertise to improve the quality of care delivered in the home. This position combines client assessments, care management, caregiver training, clinical oversight, and quality assurance . Rather than providing continuous bedside care, the RN Care Manager serves as a clinical resource for our clients, families, caregivers, and administrative team. What You'll Do Conduct initial and ongoing RN assessments and home visits for clients. Develop, review, and update individualized plans of care. Monitor clients for changes in condition, safety concerns, and changing care needs. Complete RN supervisory and quality-assurance visits. Communicate with clients, families, caregivers, providers, and the office team regarding changes in care needs. Assist with determining the appropriate level of care and caregiver skill requirements for new and existing clients. Support appropriate caregiver-to-client matching based on experience, skills, and client needs. Conduct caregiver skills assessments and competency evaluations. Provide caregiver education, coaching, and client-specific training. Participate in caregiver interviews and provide clinical input regarding candidates' skills and experience. Identify caregiver performance or competency concerns and provide retraining when appropriate. Review clinical documentation and client records for accuracy, completeness, and compliance. Assist with incident reviews, hospitalizations, changes in condition, and quality concerns. Partner with Scheduling, Client Relations, Intake, Recruiting, and Operations to support continuity and quality of care. Participate in case conferences, staff education, quality-improvement initiatives, and regulatory readiness activities. Serve as a trusted clinical resource for caregivers and office staff. What We're Looking For Current, unrestricted Registered Nurse (RN) license in the state(s) where assigned services are provided. Minimum of 2 years of nursing experience preferred . Home care, home health, geriatrics, long-term care, rehabilitation, or community-based nursing experience strongly preferred. Experience with client assessments, plans of care, caregiver education, or supervision of direct-care staff is preferred. Strong communication, teaching, documentation, organization, and critical-thinking skills. Ability to work independently while collaborating with a multidisciplinary team. Comfortable using electronic health records and healthcare technology; AxisCare experience is a plus . Valid driver's license, reliable transportation, and willingness to travel to client homes and agency locations as needed. Current CPR/BLS certification as required. Why Join Options Home Care ? $36.72 - $44.22 per hour
