Job Description

Job Summary We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to serve as a Charge Nurse, leading nursing teams in providing exceptional patient care across diverse healthcare settings. In this vital role, you will oversee daily clinical operations, coordinate patient assessments, and ensure compliance with healthcare regulations. Your leadership will foster a positive environment focused on delivering compassionate, high-quality care to patients of all ages, including those with complex medical needs such as dementia, memory care, and acute conditions. The ideal candidate is proactive, detail-oriented, and passionate about enhancing patient outcomes through expert nursing practices. Duties Supervise and coordinate care staff to ensure efficient delivery of patient care while maintaining compliance with all regulatory standards. Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication administration. Oversee medication administration adhering to HIPAA guidelines and medical documentation standards. Lead emergency response efforts during critical incidents, utilizing skills in airway management, first aid, and acute pain management. Ensure accurate electronic health records (EHR/EHR systems) management and maintain strict adherence to clinical confidentiality policies. Facilitate discharge planning and coordinate post-acute care services to support seamless patient transitions. Maintain a safe environment by following infection control protocols and ensuring proper sterile techniques during procedures like specimen collection or sterile processing. Experience Valid Licensed Practical Nurse (LPN) license with current state certification. Proven experience working in long-term care facilities, skilled nursing facilities, or hospital environments with exposure to diverse clinical settings such as ICU, emergency room, or trauma centers. Strong background in managing patients with dementia or Alzheimer's disease. Experience working with individuals with developmental disabilities or behavioral health needs is highly desirable. Proficiency in EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks for accurate documentation and medical records management. Knowledge of
ICD-9/ICD-10
coding for diagnostic evaluation and billing processes. Skilled in performing advanced procedures like tube feeding, venipuncture (phlebotomy), sterile techniques, and catheter care. Familiarity with hospice care protocols and pain management strategies for acute and chronic conditions. Ability to work effectively in fast-paced environments such as urgent care or medical-surgical units while demonstrating excellent caregiver communication skills.
