In this vital role, you will oversee daily nursing operations, coordinate patient assessments, and ensure the highest standards of clinical excellence. Responsibilities Provide direct patient care with a focus on safety, comfort, and individualized treatment plans. Supervise and support nursing staff, ensuring adherence to clinical protocols and quality standards. Coordinate patient assessments, vital signs monitoring, and medical documentation using electronic health record (EHR) systems such as Epic or Cerner. Manage medication administration, IV infusions, catheter care, and wound management with precision and attention to detail. Oversee triage processes for incoming patients or residents, prioritizing urgent needs and facilitating timely interventions. Collaborate with interdisciplinary teams to develop discharge planning, case management strategies, and care coordination efforts. Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and infection control protocols. Assist with diagnostic evaluations, specimen collection/processing, and basic life support (BLS) procedures as needed. Lead emergency response efforts in trauma or critical situations, including airway management and acute pain management. Document all patient interactions accurately within EMR/EHR systems while maintaining clinical confidentiality at all times. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current CPR certification. Proven experience working in diverse healthcare settings such as hospitals (including ICU or Level I/II trauma centers), nursing homes, or assisted living facilities. Demonstrated expertise in medical-surgical nursing, geriatrics, dementia care (including Alzheimer's and memory care), and hospice or palliative care. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with ICD coding (ICD-9/10) is highly desirable. Strong knowledge of physiology, anatomy, pharmacology related to medication administration and pain management. Ability to perform physical examinations, vital signs assessment, venipuncture (phlebotomy), catheterization, and sterile techniques confidently. Excellent communication skills for caregiver communication, patient assessment, family education, and team collaboration. Critical thinking skills for triage decision-making in urgent or emergent situations; experience working with individuals with disabilities or developmental delays is a plus.
