Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) - Charge Nurse in our Personal Care Building, where your leadership and clinical expertise will make a meaningful difference in resident care. In this vital role, you will oversee daily nursing operations and support your team to deliver exceptional health services. Your energetic approach and dedication will foster a compassionate environment focused on safety, quality, and continuous improvement. Duties Provide compassionate, respectful, and dignified care to Personal Care residents. Prepare, administer, and monitor medications according to physician orders and state regulations; maintain accurate MAR/TAR and eMAR records. Monitor and document vital signs, blood sugars, weights, oxygen levels, health status, behaviors, and changes in condition. Assess residents and report changes in health or care needs to the Nursing Manager/Supervisor. Assist with admissions, transfers, appointments, physician orders, lab and pharmacy needs, and emergency/911 calls. Maintain accurate and timely documentation in PointClickCare, medical records, and shift reports. Assist residents with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Provide guidance and coaching to CNAs, Medication Technicians, and Nursing Assistants. Coordinate outside services and transportation to ensure timely, quality resident care. Participate in resident activities, meals, safety programs, fire drills, staff meetings, training, and in-services. Complete required assessments, incident reports, and regulatory documentation in accordance with DHS and facility policies. Communicate effectively with residents, families, healthcare providers, and team members. Support Cornwall Manor's mission, Code of Conduct, policies, and commitment to quality resident care.
Qualifications:
Possession of current Practical Nurse License as licensed by Pennsylvania State Board of Nurse Examiners required.
Knowledge of medications, ability to and previous experience in assisting with medications required.
Ability to coach effectively, work proficiently, and perform as part of a team required.
Exposure to, awareness of, and desire to work with the elderly. Preferred experience of at least one year in a Personal Care/long term care setting.
Ability to remain composed in an emergency and ability to give direction and guidance under stressful situations required.
Capability of reading, writing, speaking, understanding and being understood in the English language essential.
Ability to communicate effectively and demonstrate effective listening.
Creativity and receptiveness to new methodologies; belief in merits of education, ability to display organization skills and time management skills desired. Leadership skills and effective role modeling for nursing personnel needed.
Ability to understand and carry out facility policies and procedures; and knowledge of state and federal regulations required.
Knowledge of and experience in resolution conflict.
Knowledge of team concept that utilizes person first philosophy and consensus decision making.
Valid driver's license and access to motor vehicle preferred.
$32.05 - $35.05 per hour Expected hours: 32.0 per week
