Job Description

Make a direct impact on patient independence as a Home Health Occupational Therapist, providing essential care in a field-based setting. This contract role focuses on delivering high-quality therapy services for patients in their homes, optimizing their ability to perform activities of daily living and maintain safety in their own environment.
Schedule & Assignment Details:
Full-time, 13-week contract Monday Friday, 8:00 AM 5:00
PM Caseload:
2532 points per week (57 patient visits per day) Daily travel within assigned territory (mileage reimbursement provided) Electronic documentation via Home Care Home Base (HCHB) and proficiency with OASIS assessments
Qualifications & Experience:
Active Occupational Therapist (OT) license in good standing At least 1 year of recent Home Health OT experience Experience with HCHB (preferred) and OASIS (required) Strong clinical assessment, documentation, and functional training skills Excellent communication, time management, and organizational abilities Reliable transportation, valid drivers license, and auto insurance Ability to work autonomously and coordinate with a multidisciplinary team
Key Responsibilities:
Conduct comprehensive OT evaluations and required OASIS assessments Develop individualized plans of care targeting ADLs, IADLs, and patient safety Deliver skilled interventions, therapeutic activities, and adaptive equipment training Educate patients and caregivers on independence, fall prevention, and home safety Maintain timely documentation in HCHB and communicate effectively with team members Participate in interdisciplinary meetings and maintain compliance with guidelines
