Job Description

Seeking an experienced clinician with Home Health and Occupational Therapy expertise to deliver vital care services directly in patients homes. This contract role offers a dynamic and independent work setting, focusing on helping clients regain or maintain independence in their daily lives.
Schedule/Hours:
Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Expected caseload: 2532 points per week (generally 57 patient visits per day) Field-based position with travel to patient homes (mileage reimbursement available)
Qualifications:
Current Occupational Therapist (OT) license in good standing for the state At least one year of recent Home Health Occupational Therapy experience Proficient assessment and clinical skills, with specific experience using OASIS Familiarity with electronic documentation (Home Care Home Base/HCHB preferred) Reliable transportation, active drivers license, and auto insurance Excellent communication, time management, and organizational skills Ability to excel independently in a field-based setting
Core Responsibilities:
Perform comprehensive home health OT evaluations, including OASIS assessments as required Develop and carry out individualized care plans focused on ADLs, safety, IADLs, and optimizing patient function Deliver hands-on interventions including therapeutic exercises, adaptive equipment education, and home safety/environmental modifications Monitor patient progress, update treatment plans, and ensure effective collaboration with the interdisciplinary care team Educate patients, families, and caregivers on goal achievement, compensatory strategies, and safe use of assistive devices Accurately document all care in Home Care Home Base (HCHB) Participate in case conferences, interdisciplinary meetings, and continued professional development activities
