Job Description
Solaris HealthCare seeks a compassionate Occupational Therapist to provide resident-centered care in our skilled nursing and rehabilitation centers. In this on-site role, you will evaluate functional abilities, develop and deliver individualized treatment plans, and focus on improving residents' safety, independence, and quality of life. You'll collaborate closely with an interdisciplinary team, educate families, and document outcomes while working in a supportive, patient-centered culture that values dignity, respect, and professional growth. Responsibilities Evaluate residents' functional status, mobility, and ADL performance using standardized assessments. Develop and implement individualized treatment plans to improve function, safety, and independence. Provide therapeutic interventions including ADL retraining, therapeutic exercise, neuromuscular re-education, and cognitive rehab. Collaborate with nursing, PT, speech therapy, and physicians to coordinate interdisciplinary care. Educate residents and families on home programs, adaptive techniques, and safety strategies. Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards. Recommend and train residents in use of adaptive equipment and assistive technology. Participate in care plan meetings and quality improvement initiatives. Supervise and mentor COTAs and assist with orientation of new team members, as appropriate. Ensure compliance with state and federal regulations, payer requirements, and Solaris Health Care clinical protocols.
