Vision, Dental, Medical Coverage Territory Clermont Minneola Mascotte Groveland Webster Bushnell Position Overview We are seeking a compassionate and skilled Occupational Therapist (OT) to provide home health services throughout the Clermont service area. The Occupational Therapist will assess, plan, and deliver therapeutic interventions that help patients regain independence, improve functional performance, and safely remain in their homes. The ideal candidate will possess strong clinical assessment skills, excellent time management abilities, and a commitment to delivering high-quality patient-centered care while maintaining documentation and regulatory compliance standards. Key ResponsibilitiesClinical Care & Patient Treatment Perform comprehensive occupational therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders, patient goals, and clinical assessments. Evaluate and improve patients' abilities to perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

Implement therapeutic interventions focused on:

Bathing, dressing, grooming, and toileting Meal preparation and homemaking tasks Upper extremity strength, coordination, and range of motion Cognitive and perceptual functioning Energy conservation and work simplification techniques Assess and recommend adaptive equipment, assistive devices, splints, and orthotics. Conduct home safety assessments and recommend environmental modifications to reduce falls and improve accessibility. Monitor patient progress and adjust treatment plans as needed. Develop discharge plans and recommendations for continued care. Patient & Caregiver Education Educate patients and caregivers on disease management, functional limitations, and rehabilitation goals. Teach safe transfer techniques and fall prevention strategies. Instruct patients on proper use of adaptive equipment and assistive devices. Train caregivers to support patient independence while ensuring safety and compliance with care plans. Care Coordination Collaborate with physicians, nurses, physical therapists, speech therapists, social workers, and home health aides. Participate in interdisciplinary team meetings and case conferences. Communicate significant changes in patient status to the care team promptly. Obtain and implement physician orders as necessary. Documentation & Compliance Complete evaluations, reassessments, visit notes, progress reports, and discharge documentation accurately and within required timelines. Document patient progress and response to treatment. Ensure documentation supports medical necessity and homebound status, when applicable. Maintain compliance with Medicare, Medicaid, state regulations, and agency policies. Adhere to HIPAA and patient confidentiality requirements. Follow infection prevention and control protocols. Administrative Responsibilities Manage daily schedules and travel within the assigned territory. Maintain required licensure, certifications, and continuing education requirements. Attend staff meetings, training sessions, and mandatory in-services. Consistently meet agency productivity, quality, and performance standards. Required Qualifications Current Occupational Therapist (OT) license in the state of Florida. Graduate of an accredited Occupational Therapy program. Current CPR/BLS certification. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the assigned territory. Strong clinical assessment, communication, and documentation skills. Preferred Qualifications Previous Home Health Occupational Therapy experience. Experience working with Medicare and OASIS-driven home health services. Knowledge of home health regulations and documentation requirements. Experience reducing rehospitalizations through patient education and early intervention strategies. Familiarity with interdisciplinary care coordination and care transitions.
