Job Description

Tender Touch Rehab Services seeks a compassionate Therapist for our Aviata North Fort Myers location, serving geriatric and sub-acute residents in a skilled nursing setting. In this role, you will perform evaluations, design and deliver individualized physical, occupational, or speech therapy plans, and collaborate closely with an interdisciplinary team to maximize function, safety, and quality of life. Responsibilities Provide physical, occupational, or speech therapy services to geriatric and sub-acute residents in a skilled nursing setting Perform comprehensive assessments and develop individualized treatment plans to improve function, mobility, and quality of life Deliver evidence-based interventions, monitor progress, and adjust plans in collaboration with the interdisciplinary care team Educate residents, families, and facility staff on therapy goals, home programs, and safe mobility/ADL techniques Document evaluations, treatments, and outcomes in accordance with regulatory, payer, and company standards Participate in care conferences, collaborate with nursing and medical staff, and support discharge planning Maintain clinical competencies through ongoing education and adherence to best-practice standards Contribute to a collaborative, patient-focused culture that prioritizes compassion, dignity, and high-quality resident care Required Skills Geriatric rehabilitation Skilled nursing facility (SNF) care Physical, occupational, or speech therapy treatment planning Functional and cognitive assessments Care planning and documentation (EMR) Interdisciplinary team collaboration Fall prevention and safe mobility training ADL retraining and caregiver education Regulatory and payer compliance in post-acute care Evidence-based rehabilitation interventions
