Job Description

The Home Health Occupational Therapist provides skilled rehabilitative care focused on improving a patient's functional independence, safety, and quality of life within the home setting. This role requires a clinician capable of evaluating physical, cognitive, and environmental factors, and implementing targeted interventions to restore or enhance activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Core Responsibilities Functional Rehabilitation:
Assess and treat patients to improve performance in daily living activities such as bathing, dressing, grooming, toileting, and feeding through therapeutic exercises and adaptive techniques.
Upper Extremity & Motor Function:
Develop and implement interventions to improve strength, coordination, range of motion, and fine motor skills necessary for functional independence.
Cognitive & Perceptual Training:
Evaluate and address cognitive deficits (memory, attention, problem-solving) and visual-perceptual impairments impacting safety and independence.
Home Safety & Environmental Modifications:
Conduct home assessments and recommend modifications (grab bars, adaptive equipment, fall prevention strategies) to ensure a safe living environment.
Adaptive Equipment Training:
Educate patients and caregivers on the proper use of assistive devices (e.g., reachers, dressing aids, transfer equipment) to maximize independence.
Caregiver Education:
Provide hands-on training and education to caregivers to support patient mobility, safety, and adherence to the care plan.
Clinical Assessment:
Perform comprehensive evaluations of physical, cognitive, and functional status, monitoring progress and adjusting treatment plans accordingly.
Documentation:
Maintain detailed and timely clinical documentation, including evaluations, progress notes, goal tracking, and patient response to therapy interventions.
Mandatory Qualifications Licensure:
Current, unrestricted Occupational Therapist (OT) license.
Clinical Proficiency:
Demonstrated experience in adult rehabilitation, home health, or related clinical settings.
Certifications:
Active BLS/CPR certification.
Health Screening:
Proof of a negative TB (Tuberculosis) test within the last year.
Transportation:
Reliable vehicle, valid driver's license, and active auto insurance for home visits.
Key Skills Functional Focus:
Strong ability to translate clinical findings into practical, real-world functional improvements.
Clinical Judgment:
Ability to identify safety risks, functional decline, or need for interdisciplinary intervention and escalate appropriately.
Patient & Caregiver Education:
Skilled in simplifying complex therapy techniques into clear, actionable instructions for patients and families.
Adaptability:
Ability to modify treatment approaches based on patient progress, home environment, and available resources.
Empathy & Communication:
Builds trust with patients and families while promoting independence and dignity.
Reliability:
Consistent adherence to schedules, documentation timelines, and care plan execution.
Pay:
$40.00 - $50.00 per hour
