Setting:

Skilled Nursing / Rehabilitation We are seeking an Occupational Therapist (OT/OTR) to join a rehabilitation team in Avon, Indiana. This opportunity is ideal for an OT who enjoys helping adults and seniors regain independence following illness, injury, surgery, or hospitalization.

Responsibilities:

Evaluate patients and develop individualized occupational therapy plans of care Provide interventions focused on ADLs, upper-extremity function, strength, ROM, coordination, cognition, and functional independence Recommend and provide training with adaptive equipment Establish measurable goals and monitor patient progress Collaborate closely with PT, SLP, nursing, physicians, and other members of the care team Educate patients, families, and caregivers Complete timely and accurate clinical documentation Participate in discharge planning

Qualifications:

Graduate of an accredited Occupational Therapy program Active Indiana OT license or ability to obtain SNF, LTC, rehabilitation, or geriatric experience preferred New graduates are encouraged to apply

Compensation:

$45

•55/hour For the SNF postings, this general structure is appropriate: PT, OT, and SLP are recognized specialized rehabilitation services in skilled nursing settings, and the individual therapists must hold the applicable professional credentials.
