Others discover that they're especially good at something different. Admissions. Walking into a new patient situation. Conducting a comprehensive assessment. Asking the right questions. Completing OASIS accurately. Identifying risks. Establishing the baseline. Organizing a complicated clinical picture and determining what needs to happen next. If that's where you're strongest, why not build a role around it? Home Health Care, Inc. is hiring an experienced Occupational Therapist (OTR/L) to focus on Start of Care admissions throughout the Twin Cities Metro and surrounding communities. Make Admissions Your Specialty This isn't a traditional OT position built around carrying the same treatment caseload week after week. Your focus is the front end of home health care . You'll meet patients as they're entering services, complete comprehensive Start of Care assessments, gather the clinical and functional information the team needs, complete OASIS documentation, and establish an appropriate plan for what comes next. Then you'll move on to the next admission. For an OT who enjoys assessment more than repetition , that can be an entirely different way to use your license. You need to gather a lot of information quickly without making the patient feel like they're being interrogated. You need to understand the referral while independently verifying what's actually happening. You need to evaluate function, cognition, safety, caregiver support, environmental barriers, and clinical needs. And then you need to translate all of it into accurate documentation and an actionable plan of care. The more admissions you complete, the sharper those skills become. What You'll Be Doing Complete comprehensive Start of Care Occupational Therapy evaluations Complete required OASIS assessments and admission documentation Establish functional baselines and patient-centered goals Assess ADLs, cognition, functional mobility, upper-extremity function, caregiver support, and home safety Identify fall risks and barriers to safe independence Determine immediate functional and environmental priorities Develop individualized plans of care Recommend adaptive equipment and home modifications when appropriate Provide initial patient and caregiver education Coordinate with nursing, PT, SLP, social work, physicians, and other clinicians Complete accurate and timely electronic documentation Travel throughout the Twin Cities Metro and surrounding service area based on admission needs We're Looking for Someone Who Already Understands Home Health Because of the specialized nature of this position, previous experience matters. We're especially interested in an Occupational Therapist who already understands home health workflow, OASIS, comprehensive assessments, Medicare requirements, and Start of Care documentation . You should be comfortable entering unfamiliar patient environments, establishing rapport quickly, managing your time independently, and completing detailed documentation accurately. What You Bring Current Minnesota Occupational Therapist (OTR/L) license Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong assessment and clinical reasoning skills Excellent documentation and organizational skills Strong patient, caregiver, physician, and interdisciplinary communication Confidence working independently in patient homes Especially Valuable Experience Previous home health experience OASIS experience Start of Care or admission experience Medicare home health knowledge Experience with medically complex, orthopedic, neurological, or geriatric patients Optional Weekends Create Additional Earning Opportunity Your primary schedule is during the week, with Start of Care visits paying $100 per admission plus mileage reimbursement . Want to pick up more? Optional weekend Start of Care visits may be available at $120 per admission . You can take advantage of the additional earning opportunity when it fits your availability without making regular weekend work the foundation of the position.
