Job Description

Liberty Health seeks an Occupational Therapist to provide person-centered, evidence-based therapy services within innovative behavioral health and human services programs. You will evaluate functional abilities, develop and implement individualized treatment plans, and deliver therapeutic interventions that promote independence, safety, and community participation. Collaborate closely with interdisciplinary teams, document outcomes, and contribute to continuous quality improvement. This role partners with public-sector agencies and diverse populations, ensuring compassionate, accountable care aligned with best clinical and regulatory practices. Responsibilities Conduct occupational therapy evaluations and functional assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic interventions to improve daily living and vocational skills Collaborate with interdisciplinary teams and public-sector partners Document services, progress notes, and measurable outcomes accurately Educate individuals, families, and caregivers on strategies and adaptive techniques Ensure compliance with regulatory, ethical, and organizational standards Contribute to quality improvement and data-driven care initiatives Adapt interventions for diverse populations and complex behavioral needs Participate in team meetings, case conferences, and staff trainings Required Skills Occupational therapy evaluation and treatment planning Functional and psychosocial assessment Documentation and electronic health records (EHR) Interdisciplinary team collaboration Knowledge of behavioral health and developmental disabilities ADL/IADL and sensory integration interventions Client and caregiver education Regulatory and ethical compliance Outcome measurement and data-driven practice Crisis prevention and de-escalation techniques
