Bridgeway Care at Home, a premier provider of outpatient therapy services in senior living communities, is seeking a Full-Time Occupational Therapist to join our growing in-house therapy team serving Assisted Living (ALF) and Independent Living (ILF) residents. Our therapists provide Medicare Part B outpatient occupational therapy services within Assisted Living and Independent Living communities, helping residents maintain independence, improve function, and maximize quality of life. Responsibilities Evaluate residents and develop individualized occupational therapy treatment plans based on their functional needs and goals. Provide skilled outpatient occupational therapy services under Medicare Part B. Address deficits affecting activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility, safety, cognition, and overall functional independence. Select and implement evidence-based interventions to improve strength, coordination, cognition, balance, upper extremity function, and participation in daily activities. Recommend adaptive equipment and train residents, caregivers, and community staff in its safe and effective use. Assess residents' living environments and recommend modifications, compensatory strategies, and safety interventions to improve accessibility and reduce fall risk. Educate residents, families, caregivers, and community staff on home exercise programs, energy conservation, fall prevention, and strategies to maximize independence. Monitor resident progress, modify treatment plans as appropriate, and document services accurately and timely in accordance with Medicare Part B regulations. Communicate and collaborate with physicians, nurses, case managers, community staff, and other healthcare professionals to ensure coordinated resident care. Participate as appropriate in interdisciplinary care planning and communication regarding residents' functional status and therapy needs. Qualifications Current, active Occupational Therapist license in the state of practice. Graduate of an accredited Occupational Therapy program. Excellent communication, organization, and clinical reasoning skills. Ability to work effectively with older adults in Assisted Living and Independent Living environments. Senior living, outpatient, skilled nursing, or home health experience is preferred but not required. New graduates are encouraged to apply.
