Job Description

Step into a rewarding role delivering essential occupational therapy services in a home health setting, supporting patients in maximizing their independence and improving daily living skills. As a valued Home Health Occupational Therapist, you will make a direct impact through hands-on care and personalized treatment plans, collaborating closely with fellow healthcare professionals.
Schedule:
Monday through Friday, 8:00 AM to 5:00 PM Full-time, 13-week contract
Desired Experience and Qualifications:
Active Occupational Therapist license in good standing within the state At least one year of recent experience in Home Health Occupational Therapy, with field-based experience preferred Strong background with OASIS assessment and documentation Experience with Home Care Home Base (HCHB) preferred Skilled in clinical assessment, functional training, and education for both patients and caregivers Reliable transportation, valid drivers license, and current auto insurance Ability to work autonomously, demonstrating excellent organization, time management, and communication
Key Responsibilities:
Conduct thorough home health occupational therapy evaluations, utilizing OASIS as needed Develop and implement tailored care plans focused on ADLs, upper extremity rehabilitation, cognition, and safety in the home environment Provide interventions such as self-care training, therapeutic activities, and home modification recommendations Assess for and recommend adaptive and durable medical equipment Update and modify treatment protocols as patient needs evolve, and communicate regularly with the interdisciplinary team Educate patients and families on functional improvement, safety, and use of adaptive aids Complete accurate documentation within Home Care Home Base (HCHB) Participate in case conferences, team meetings, and ongoing professional education Benefits include weekly pay, access to medical, dental, and vision insurance, 401(k) plan options, 24/7 customer support, mileage reimbursement, and opportunities for contract extensions.
