Full-Time | W-2 | On-Site Occupational Therapist Opportunity ND Global Consulting Services, Inc. (NDGCS) is seeking a dedicated and experienced Occupational Therapist (OT) to join a 252-bed Skilled Nursing and Rehabilitation Facility in Montrose, New York. This is an excellent opportunity for an Occupational Therapist who is passionate about helping residents achieve their highest level of independence and functional ability. Position Responsibilities Evaluate resident function and develop individualized occupational therapy treatment plans. Provide therapy services focused on activities of daily living (ADLs), cognition, adaptive techniques, upper-extremity function, and fine motor skills. Assess wheelchair seating, positioning systems, orthotics, and adaptive equipment needs. Perform home visits, discharge planning, environmental assessments, and caregiver education. Collaborate with interdisciplinary teams including nursing staff, physicians, families, and rehabilitation professionals. Participate in care plan meetings and contribute to resident-centered treatment goals. Supervise Certified Occupational Therapy Assistants (COTAs) as required. Maintain accurate and timely documentation in the electronic health record system. Minimum Qualifications Current and unrestricted New York State Occupational Therapist License. Minimum of three (3) years of Occupational Therapy experience. Strong clinical, communication, and documentation skills. Ability to work effectively within a multidisciplinary healthcare team. Preferred Qualifications Skilled Nursing Facility (SNF) experience. Long-Term Care, Geriatric Rehabilitation, Memory Care, Wheelchair Seating, or Discharge Planning experience. Experience working with adaptive equipment and positioning systems.
