Job Description

Occupational Therapist for Home Health | Trumbull County Expand Health is a family owned and operated business that is rapidly growing! We seek to attract and retain the best staff who share in our dream to stand out in the lives of patients, our community, and healthcare as a whole The Occupational Therapist (OT) is a member of a multidisciplinary team at Expand Health at Home. The OT helps individuals improve their ability to perform everyday activities in accordance with company standards and federal, state, and local standards, guidelines, and regulations. The OT's goals are to rehabilitate and educate the patient to regain and retain their independence in their home setting.
OT Responsibilities:
Conduct comprehensive patient evaluations to determine functional limitations, impairments, and needs. Develop individualized plans of care that establish measurable, functional goals and appropriate treatment strategies. Implement evidence-based therapeutic interventions to address physical, cognitive, and psychosocial deficits, adapting treatment as the patient's condition changes. Monitor and document patient progress toward goals, adjusting the treatment plan in collaboration with the patient and interdisciplinary team. Educate patients, families, and caregivers on diagnoses, prognosis, home exercise programs, adaptive techniques, and the use of assistive devices. Collaborate with physicians and other healthcare professionals (e.g., physical therapists, speech-language pathologists, social workers) to ensure a cohesive and comprehensive interdisciplinary approach to care. Ensure timely and accurate documentation of all patient encounters, evaluations, treatment plans, and progress in compliance with regulatory and organizational standards. Assess the home environment to recommend modifications and durable medical equipment that enhance patient safety and independence. Participate in quality improvement initiatives and contribute to the enhancement of department processes and overall patient outcomes. Manage workload efficiently to meet or exceed established productivity targets without compromising the quality of care

Qualification, Education and Experience:
Requires a Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program. Must possess and maintain a valid, unrestricted Occupational Therapy license in all applicable states of practice. Commitment to continuous professional development and ongoing education to ensure licensure renewal and stay current with industry standards

Base Pay:
$90,000.00 - $110,000.00 per year
