Supportive leadership and collaborative therapy team Position Summary We are seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to maximize independence, safety, and quality of life. Key Responsibilities
Evaluate residents and develop individualized occupational therapy treatment plans
Provide skilled occupational therapy services in accordance with physician orders and established goals
Monitor resident progress and modify treatment plans as appropriate
Communicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team members
Maintain accurate and timely documentation in accordance with regulatory and payer requirements
Participate in care planning meetings, rehabilitation conferences, and discharge planning activities
Educate residents, families, and caregivers regarding treatment goals and home programs
Conduct in-service education and training for facility staff as needed
Ensure compliance with all state practice acts, facility policies, and regulatory requirements
Promote resident safety, independence, and quality of life Qualifications Education & Experience
Bachelor's degree in Occupational Therapy required
Master's or Doctorate degree in Occupational Therapy preferred
Experience in skilled nursing, rehabilitation, or long-term care preferred
Experience with PointClickCare (PCC) and Casamba preferred
Strong communication, clinical, and documentation skills Licenses & Certifications
Current Occupational Therapist license in South Carolina in good standing
Must maintain all required continuing education requirements
