Jonesborough, TN Jonesborough, Tennessee, is the state's oldest town (est. 1779), known as the "Storytelling Capital of the World," featuring a well-preserved historic downtown with 18th- and 19th-century architecture, and serving as a charming heritage tourism destination in the Johnson City metropolitan area. ️ Responsibilities Review clinical information and evaluate its impact on the assessment and treatment process. Conduct comprehensive functional assessments using standardized procedures. Develop and implement individualized treatment plans with achievable functional goals. Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly. Revise treatment plans based on patient performance and progress. Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care. Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover. Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes. Assist with discharge planning and maintain knowledge of community resources. Participate in care conferences, meetings, and quality improvement initiatives. Maintain clinical competence through continuing education opportunities. Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines. Support the company's mission, vision, and professional expectations.
