Job Description

CenterWell Home Health seeks a PRN Occupational Therapist to provide compassionate, in-home therapy for seniors and adults with chronic conditions. In this role, you will perform comprehensive assessments, create individualized treatment plans, and deliver hands-on interventions that improve safety, function, and independence at home. You'll collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using electronic systems.
