Job Summary We are seeking a dedicated and energetic Occupational Therapist (OT) to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized occupational therapy programs that enhance patient independence and quality of life across diverse settings. This position is to work with adults in acute care, or individuals with neurodevelopmental disorders, you will play a key role in delivering evidence-based interventions that promote functional recovery and well-being. Your expertise will support clients through their rehabilitation journey, discharge planning, and ongoing care management, ensuring comprehensive patient-centered care. Duties Conduct thorough occupational therapy assessments to evaluate patients' physical, cognitive, and emotional needs. Develop individualized treatment plans aligned with clinical goals, medical documentation standards, and evidence-based practices. Implement targeted occupational therapy programs to improve skills related to activities of daily living (ADLs), mobility, sensory processing, and fine motor skills. Assist patients with ADLs such as dressing, grooming, feeding, and toileting to foster independence. Collaborate with interdisciplinary teams—including speech therapists, physicians, case managers, and educators—to coordinate comprehensive care strategies. Support discharge planning by preparing clients for transition home or to other care settings through detailed recommendations and caregiver training. Maintain accurate clinical documentation within electronic health records (EHR), ensuring compliance with healthcare regulations and medical terminology standards. Provide specialized interventions for geriatric populations in skilled nursing facilities. Educate clients and families on therapeutic exercises, adaptive techniques, and safety measures to promote ongoing progress outside of therapy sessions. Participate in case management activities to monitor progress, adjust treatment plans as needed, and support client communication throughout the therapy process. Experience Proven experience working with adults with disabilities across various environments such as inpatient hospitals, skilled nursing facilities, outpatient clinics and/or home health settings. Experience conducting occupational therapy assessments using standardized tools and developing effective treatment plans based on physiology knowledge and anatomy understanding. Prior involvement in post-acute care environments including ICU or acute care units is highly desirable. Skilled in medical documentation practices utilizing electronic health records (EHR) systems while adhering to clinical documentation standards. Knowledge of medical terminology related to rehabilitation medicine enhances communication within interprofessional teams. Ability to implement occupational therapy programs tailored for geriatric population and adapt interventions based on client needs and evidence-based practice guidelines. $40.00 - $60.00 per hour Expected hours: 2.0 - 40.0 per week
