Job Description

Job Overview Gardner Rehabilitation and Nursing Center is seeking compassionate and dedicated PRN Physical Therapists (PTs), Physical Therapist Assistants (PTAs), and Certified Occupational Therapy Assistants (COTAs) to join our rehabilitation team. This PRN position offers flexibility for experienced therapy professionals who enjoy working in a skilled nursing and rehabilitation environment. Our therapists play an important role in helping residents improve strength, mobility, function, and independence while supporting their overall quality of life. The ideal candidate is compassionate, dependable, clinically skilled, and committed to providing individualized, patient-centered care. Responsibilities Evaluate patients/residents and develop individualized treatment plans based on their functional needs and goals. Provide appropriate physical or occupational therapy interventions to improve mobility, strength, balance, coordination, and independence. Deliver safe, effective, and evidence-based rehabilitation services in accordance with professional standards and applicable regulations. Monitor patient progress and modify treatment plans as appropriate. Assist patients with transfers, mobility, positioning, and therapeutic activities while maintaining safety at all times. Collaborate with nursing staff, physicians, and other members of the interdisciplinary team to coordinate patient care. Educate patients and caregivers on exercises, mobility techniques, safety, and strategies to promote independence. Maintain accurate and timely documentation of evaluations, treatments, progress, and outcomes. Follow all facility policies, safety procedures, and applicable federal and state regulations. Provide compassionate care while treating every resident with dignity, respect, and professionalism. Qualifications Current Massachusetts license or certification appropriate to the position as a Physical Therapist (PT), Physical Therapist Assistant (PTA), or Certified Occupational Therapy Assistant (COTA) . Experience in skilled nursing, rehabilitation, long-term care, or a similar healthcare setting preferred. Strong clinical assessment, communication, and documentation skills. Ability to work collaboratively as part of an interdisciplinary team. Commitment to providing person-centered care and promoting resident independence. Ability to safely assist patients with mobility, transfers, and therapeutic activities. Dependable and flexible availability to work on an as-needed/PRN basis. Why Join Our Team? Pay:
$38.00 - $60.00 per hour
