Job Description

South Pacific Rehab Services is seeking an Occupational Therapy Assistant to provide compassionate, resident-centered care in skilled nursing and post-acute settings. Working under the direction of an Occupational Therapist, you will deliver one-on-one treatments that focus on functional recovery, ADLs, safety, and quality of life. Responsibilities include implementing care plans, documenting in EMR, collaborating with an interdisciplinary team, and educating residents and families. Responsibilities Provide one-on-one occupational therapy treatments under OT supervision in skilled nursing and post-acute settings Assist with evaluations and implement individualized treatment plans focused on function and safety Document treatments, progress notes, and outcomes accurately in the EMR system Educate residents and families on exercises, adaptive techniques, and use of assistive equipment Collaborate with an interdisciplinary care team to support resident-centered goals and discharge plans Ensure safe treatment environments, adhering to infection control and facility policies Participate in case conferences, in-services, and ongoing clinical education Support a clinician-centered, ethical, and patient-first culture Required Skills Occupational therapy treatment implementation Functional mobility and ADL retraining Treatment documentation and EMR use Therapy exercise and activity progression Interdisciplinary team collaboration Patient and family education Care planning in SNF/post-acute settings Vital signs and patient safety monitoring Time management in caseload management Infection control and standard precautions
