South Pacific Rehab Services is seeking a Licensed Occupational Therapist to provide resident-centered care in skilled nursing and post-acute facilities. In this on-site role, you will complete evaluations, develop individualized treatment plans, and deliver one-on-one interventions to improve ADLs, functional mobility, safety, and quality of life. You will collaborate closely with an interdisciplinary team, document outcomes, and educate patients and families. Responsibilities Perform comprehensive OT evaluations for residents in skilled nursing and post-acute care settings Develop and implement individualized, function-focused treatment plans Provide one-on-one interventions to improve ADLs, mobility, safety, and quality of life Collaborate with an interdisciplinary team on care plans, goals, and discharge planning Document care thoroughly in the EMR and track clinical outcomes Educate patients, families, and facility staff on rehab strategies and equipment use Contribute to a clinician-centered culture that values ethics, respect, and open communication Participate in continuing education and clinical mentoring opportunities Required Skills Occupational therapy evaluation and treatment
