Job Description

Signature HealthCARE is seeking a compassionate Certified Occupational Therapist Assistant to support resident-centered rehabilitation in our long-term and post-acute care community. Working under the direction of an Occupational Therapist, you will help evaluate residents' functional abilities, carry out individualized treatment plans, and train residents in ADLs, mobility, and use of adaptive equipment. You'll document progress, collaborate with an interdisciplinary care team, and educate families to promote safety and independence. Responsibilities Implement OT treatment plans under supervision of an Occupational Therapist Assist with functional assessments and objective measurements Provide ADL, mobility, and cognitive retraining to residents Train residents on safe use of adaptive equipment and devices Accurately document treatments, progress, and resident response in EMRCollaborate with nursing, therapy, and medical staff on care plans Educate residents and families on home safety and carryover strategies Support discharge planning to maximize safety and independence Follow facility, state, and federal regulations and ethical standards Participate in in-service training and quality improvement initiatives Required Skills Occupational therapy interventions Activities of daily living (ADL) training Therapeutic exercise and functional mobility Use and fitting of adaptive equipment Gait and balance training Patient and family education Clinical documentation and EMR use Interdisciplinary care coordination Safety assessment and fall prevention Time management and caseload prioritization
