Job Description
Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety. You'll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers. Responsibilities Evaluate senior patients in skilled nursing and post-acute settings using standardized assessments Develop and implement individualized, evidence-based treatment plans to improve function and independence Provide ADL, mobility, cognitive, and safety training tailored to seniors' needs and goals Document evaluations, treatment sessions, and progress notes accurately in EMR systems Collaborate with PT, SLP, nursing, and medical staff on interdisciplinary care plans Educate patients, families, and caregivers on home programs, adaptive equipment, and safety Participate in case conferences and quality improvement initiatives focused on outcomes Mentor and support therapy teammates in a collaborative, patient-centered culture Required Skills Geriatric occupational therapy
