You recognize when something that used to require significant assistance suddenly requires only a little. You know when it's time to challenge them further, and you also recognize when something has changed that the supervising OTR/L needs to know about. That consistency matters. Your visits help patients practice, adapt, gain confidence, and gradually reclaim pieces of their independence. You're helping them practice until they can. The plan may call for improved coordination so a patient can return to meal preparation. You're working through the activities that build those skills. A caregiver may need education on how to safely assist without doing everything for the patient. You're there reinforcing those strategies. You're not simply completing treatment visits. You're helping patients build on what they accomplished during the visit before. What You'll Be Doing Implement individualized treatment plans developed by supervising Occupational Therapists Provide skilled one-on-one Occupational Therapy treatment in patient homes Address ADLs and functional independence Provide fine motor, coordination, strengthening, cognitive, and functional activities Reinforce adaptive and compensatory strategies Educate patients and caregivers on safety and functional techniques Monitor patient response and progress between visits Communicate clinical observations and changes to the supervising OTR/L Collaborate with PTs, PTAs, nurses, and other interdisciplinary clinicians Complete accurate and timely clinical documentation Your COTA Background Can Translate to Home Health We're interested in COTAs from Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Assisted Living, Memory Care, Long-Term Care, Outpatient Therapy, and Home Health. What matters is your ability to connect with patients, provide strong hands-on treatment, recognize meaningful changes, and work collaboratively with your supervising Occupational Therapist. What We're Looking For Active Minnesota Certified Occupational Therapy Assistant license Valid driver's license and insured, reliable transportation Strong patient and caregiver communication Ability to independently manage a field-based schedule Strong organization and documentation skills Comfortable traveling within an assigned Southwestern Minnesota territory Home health experience preferred Experience with geriatric or medically complex patients welcomed Be There for the Small Wins That Become Big Ones Recovery isn't always dramatic.
