The Infusion Pharmacist is responsible for dispensing IV drugs prescribed by physicians and other health care practioners and providing information to patients about medications and their use. The pharmacist may advise physicians, other health practioners, and patients on the selection, dosage, interactions, and side effects of medications.
Provide clinical pharmacy services for in home infusion therapy, collaborating with healthcare professionals to optimize patient care.
Conduct patient assessments to identify medication-related problems and develop appropriate pharmaceutical care plans.
Monitor and adjust medication therapy to ensure optimal outcomes and minimize adverse effects.
Participate in multidisciplinary rounds and provide drug information to healthcare team members.
Provide education and counseling to patients on medication use, side effects, and adherence.
Contribute to the development and implementation of pharmacy policies and procedures.
Participate in quality improvement initiatives to enhance patient safety and medication management processes.
Qualifications:
Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
Completion of a pharmacy residency program, preferably in a clinical or acute care setting.
Mix and compound IV medications from a Flow Hood.
Compounding medication using proper aseptic techniques.
Post-residency experience as a clinical pharmacist is highly desirable.
Strong knowledge of, pharmacokinetics, pharmacodynamics, and physiology.
Proficiency in medication therapy management and evidence-based medicine practices.
Experience with managed care principles and formulary management is a plus.
Ability to provide IV infusion therapy and perform medication administration as needed.
Excellent communication skills to effectively collaborate with healthcare professionals and educate patients.
