Full Job Description Overview:
We are a privately owned, 24-hour long-term care pharmacy serving skilled nursing and assisted living facilities throughout Central California. Our pharmacy is committed to high clinical standards, regulatory compliance, and dependable service for our partner facilities. Job Summary We are seeking a highly skilled Overnight Clinical Pharmacist specializing in Long-Term Care to join our healthcare team. This full-time overnight role is ideal for a licensed California pharmacist who is confident working independently during late hours while maintaining strong clinical judgment and accuracy. The Overnight Clinical Pharmacist plays a critical role in ensuring timely medication verification, clinical oversight, and continuity of care for long-term care patients.
Schedule:
Rotating 7 nights on / 7 nights off Responsibilities Conduct comprehensive medication reviews and patient assessments to develop individualized treatment plans. Collaborate with healthcare providers to optimize medication therapy Review and verify medication orders for accuracy, clinical appropriateness, and regulatory compliance Supervise overnight dispensing and medication preparation for long-term care facilities Perform sterile compounding when required, including IV medications and antibiotics Serve as a clinical resource for nursing staff and prescribers during overnight hours Monitor medication therapies and identify opportunities for optimization or intervention Ensure accurate documentation in accordance with California Board of Pharmacy standards Support efficient overnight workflow in collaboration with technicians and on-call staff Maintain a high standard of patient safety, timeliness, and professionalism Skills Strong knowledge of physiology, anatomy, medical terminology, and pharmacology related to acute care and pain management. Proven expertise in managed care, medication administration, IV infusion techniques, and aseptic procedures. Critical thinking skills with the ability to handle complex clinical scenarios involving acute pain management and medication therapy adjustments. Excellent communication skills for effective collaboration within multidisciplinary teams. Strong clinical knowledge and sterile compounding competency Excellent attention to detail and decision-making skills Ability to work independently with minimal supervision Working knowledge of pharmacotherapy and medical terminology
Qualifications:
Pharm.
D. degree or equivalent Active, unrestricted California Pharmacist license Prior pharmacist experience required (long-term care and geriatric experience preferred) Interested candidates are encouraged to submit their resume through Indeed.
