This vital role helps ensure patients understand their medication regimens and are more likely to adhere to them, supporting healthier recoveries. Responsibilities include working closely with patients prior to discharge to review and counsel on their medications, coordinating with healthcare teams to enable seamless medication access, and participating in ambulatory care initiatives across a large health system. This contract position supports a prominent county health system known for a wide range of outpatient clinics and hospitals. Preferred candidates are recent PGY1 residents, but those without postgraduate residency training who have retail or outpatient pharmacy experience will also be considered. Fluency in Spanish is a valuable asset for enhancing communication and patient engagement.
