Pharmacist Reports to:

Pharmacy Manager Job Purpose:

The Pharmacist ensures that Pharma Fusion operates in full compliance with all applicable laws and regulations. This role is responsible for simple compounding (ex. Magic mouthwash) and dispensing prescribed medications, providing functional supervision to professional, paraprofessional, and support staff, and performing related duties as required. Future responsibilities may include infusion services, sterile preparations, and compounding.

Key Responsibilities:

Regulatory Compliance:

Maintain records of all pharmacy transactions to ensure accurate control and accountability for all medications, as required by state and federal laws. Ensure that the pharmacy operates legally, meeting all inspection requirements and adhering to state and federal regulations governing pharmacy operations.

• Prescription Fulfillment: Accurately fill prescriptions for pharmaceutical preparations and devices for patients at company facilities.

Maintain detailed records of all prescription transactions, submit appropriate claims for third-party reimbursement, and consult with physicians and nurses regarding pharmaceutical queries. Provide advice on formulary medications, cost-effective therapies, and innovative treatment options.

Staff Supervision:

Provide functional supervision to professional, paraprofessional, and support staff in the pharmacy. This includes training, assigning duties, and contributing to performance evaluations.

Error Reporting and Prevention:

Document and report medication errors to the direct supervisor. Implement error reduction and prevention strategies based on errors and ISMP (Institute for Safe Medication Practices) standards. Maintain an accurate error log and adherence log.

Specialty Responsibilities:

o Manage packing and unpacking of specialty medications. o Create and send order breakdown.

Documentation and Reporting:

o Prepare and manage the ship error/accuracy log. o Oversee the temp/cleaning log o Maintain the on-call log. o Track and report measurements for URAC, ACHC, NABP and custom reporting requirements. o Conduct and document 3rd party pharmacy audits. o Oversee newsletter responsibilities. o Complete initial and follow-up assessments for clinical patients as required. o Assist with the upkeep of the Asembia platform in collaboration with other pharmacy staff.

Supply and Inventory Management:

o Pull and order necessary supplies. o Manage drug ordering processes.

Operational Duties:

o Participate in the on-call rotation. o Participate in the weekly trash rotation. o Handle fax machine duties. o Contribute to the monthly rotation for patient education topics. o May be required to obtain additional professional certifications or engage in ongoing training programs as needed.

Legal and Ethical Compliance:

Uphold all applicable federal and state laws and adhere to the highest ethical standards. Legal and ethical compliance is a fundamental responsibility of this role.

Additional Duties:

Perform other related duties as assigned by pharmacy manager.

Standards Adherence:

Serve and protect the company by adhering to professional standards, company policies and procedures, and relevant federal, state, and local requirements, including URAC (Utilization Review Accreditation Commission) standards of care.

Required Education, Training, and Professional Experience:

• Licensed Pharmacist with a PharmBS/PharmD from an accredited college of Pharmacy.
• Responsible for maintaining Continuing Education (CE) requirements.
• Responsible for obtaining and maintaining licensure in all states that require. Pharmacists will be given a reasonable amount of time to obtain licenses. (2 test per month) Skills and Qualifications:

Pay:

$56.00 - $67.44 per hour Expected hours: 40.0 per week
