Job Description

SSM Health seeks a Clinical Pharmacist to provide patient-centered medication management within our integrated Catholic, not-for-profit health system. In this role, you will review and verify medication orders, perform clinical pharmacokinetic dosing, monitor for drug interactions and adverse effects, and collaborate with physicians and nurses to optimize therapy. You will educate patients and staff, support formulary and stewardship initiatives, and uphold SSM Health's mission of compassionate, high-quality care while growing professionally in a supportive environment. Responsibilities Review, verify, and dispense medication orders for safety, accuracy, and appropriateness Provide clinical dosing, pharmacokinetic monitoring, and therapeutic drug management Perform medication reconciliation and monitor for interactions and adverse drug events Collaborate with physicians, nurses, and care teams to optimize medication therapy Educate patients and clinical staff on medications, indications, and safe use Support antimicrobial stewardship and formulary management initiatives Ensure compliance with regulatory, accreditation, and safety standards Document clinical interventions and outcomes in the electronic health record Participate in quality improvement and medication safety projects Precept or mentor pharmacy learners as assigned Required Skills Clinical pharmacy practice Medication therapy management Pharmacokinetics and dosing Sterile compounding Drug interaction screening Electronic health records (EHR) Medication reconciliation Antimicrobial stewardship Patient and staff education Regulatory and safety compliance
