Daviess Community Hospital Job Description The Pharmacist is responsible for accomplishing the daily functions performed by the Pharmacy Department in accordance with departmental policies and procedures. The Pharmacist also assists the Pharmacy Director in performing many of the management functions of the department. Actively verify prescriptions with a focus on speed and zero-error accuracy. Evaluate patient profiles for correct dosages and clinical appropriateness before releasing orders to the fulfillment queue. Work with a team to resolve immediate operational bottlenecks, such as missing prior authorizations, insurance rejections, or inventory discrepancies. Work as part of the team during peak volumes to count, pack, and prepare complex or hazardous specialty medications for shipment. Provide direct, efficient clinical counseling to patients via Medication Therapy Management Consults and answering patient calls regarding complex therapies. Communicate with outside vendors and pharmacies to provide patient-centered care. Maintains accurate records of all department transactions. Purchases pharmaceuticals and maintains adequate supplies of all item. Assures that pharmaceuticals are stored properly in all areas of the hospital. Prepares and dispenses medications (including compounded medication), IV admixtures, and chemotherapy products. Provides age-appropriate care to all DCH patients. Reviews and interprets all physician orders for appropriateness and clarifies all concerns, issues with the MD before dispensing medication. Monitors patient drug therapy to assure safe, effective, and rational therapy. Qualifications Bachelor of Science degree in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Currently licensed by the Indiana Board of Pharmacy with one to two years of hospital pharmacy practice, preferably in a hospital setting. In addition to Indiana, licensure in Florida, South Carolina, Illinois, California, Michigan is preferred. Thorough knowledge of pharmacy practice, preferably in a hospital setting. 340b experience preferred
