Job Summary:

Coordinates Home Infusion for patients needing to go home on IV medication from our system hospitals, Participates in program development, quality assurance and delivery of care, Remains familiar with pumps, access devices, drugs and IV access, Develops and maintains relationships with referrals services and client base, Assists our clinical team by providing documents and information necessary for discharge, Acts as a resource to case management,

Education Required:

Associate degree Preferred or expertise as a pharmacy technician

Additional Education Preferred:

Previous Home Care Experience Desired Insurance Knowledge a plus

Licensure Required:

Home Infusion experience preferred Job Responsibilities (list 8-12): 1, Assist with coordination of care between the referral source, the pharmacy, the nurse department and patient/caregiver, 2, Develops, maintains, and grows home infusion admissions through positive referral source relationships and responsive care and customer service, 3, Discuss insurance benefits and co-pay information with the patient or responsible party 4, Helps
