We are currently seeking a Full Time Clinical Pharmacist. This position will work throughout all of locations in Williamsville, Lockport, Orchard Park and Downtown Buffalo. .

• 4:30pm Job duties included, but are not limited to: Works collaboratively with providers to provide diabetes education.

Works collaboratively with providers to provide medication therapy management. Provide assistance with medication reconciliation. Provides any necessary drug information as requested. Scheduling of any needed appointments for patients.

Qualifications:

Graduate of an ACPE Accredited Pharmacy School. Holds or is eligible for NYS licensure (with Immunization rider). Completion of a PGY1 or PGY2 (preferred) Residency Training Program or 5 years experience in a relevant practice setting.
