We specialize in Long Term Care Facilities including Skilled Nursing Homes, Assisted Living Facilities, Group Homes and Homes for the Developmentally Disabled.

Full Time Duties/Responsibilities:
1. Assist and educate nursing staff regarding their patient's current drug therapy 2. Problem solve regarding drug therapy for nursing staff (including contacting the doctor for their patient) so they can concentrate on direct patient care 3. Perform prospective and retrospective Drug Utilization Review for new medication orders and admissions for skilled nursing and assisted living facilities 4. Comply with all applicable company policies and procedures 5. Comply with all state and federal regulations for pharmacy practice 6. Comply with all HIPAA guidelines for patient care 7. Assist in maintaining records for controlled substances 8. Develop knowledge of DME products and services that the pharmacy provides to assisted living facilities 9. Maintain emergency boxes and records for emergency boxes 10. Assist the pharmacy manager with continuous quality improvement initiatives to meet all state, federal, and accreditation standards. 11. Review all medication orders from doctors for accuracy and completeness 12. Check Cycle fill medications for accuracy and completeness
Minimum Qualifications:
Graduate from an accredited school of pharmacy from the American Board of Pharmaceutical Education. PharmD and/or experience in clinical pharmacy or long-term care pharmacy preferred Effective verbal and written communication skills
Required Licensure/Certification:
Current State Pharmacist License, Must be licensed in
SD Competency Requirements:
Completing annual CE requirements and understanding of new clinical trends in long-term care pharmacy practice
Physical Requirements:
1. Sitting, standing, walking; at desk and throughout the pharmacy 2. Operation of automation, office machinery within the pharmacy 3. Occasionally lift/move items up to 50 pounds 4. Work at desk on the computer for extended periods of time
