Key Responsibilities:

Review medication orders to assess drug therapy appropriateness, dosage, regimen, and potential drug interactions. Provide pharmacokinetic consultations and anticoagulation dosing support. Engage actively in clinical services, including medication reconciliation and discharge medication education. Oversee pharmaceutical compounding, ensuring compliance with USP regulatory standards, focusing on USP 797 and USP 800 guidelines. Utilize EPIC electronic health records to manage medication orders and documentation efficiently.

Qualifications and Desired Experience:

Current pharmacist license with relevant state credentials. Proven experience in hospital pharmacy settings. Strong knowledge of pharmacokinetics, anticoagulation therapy, and pharmaceutical compounding regulations. Familiarity with USP 797 and USP 800 standards. Proficiency with EPIC electronic health systems preferred.
