We specialize in serving patients with schizophrenia, bipolar disorder, substance use disorder, and related challenges - patients who are often overlooked by traditional healthcare systems. We provide advanced pharmacy services and wraparound care that ensure patients can access and stay on complex medications. We offer in-home and community-based injection support, help patients navigate insurance and prior authorizations, and work closely with providers to overcome barriers to treatment. By strengthening connections between patients, care teams, and support systems, we make it possible for people living with behavioral health challenges to achieve stability and long-term success. Job Overview The Clinical Pharmacy Specialist executes patient-facing and program-specific work that supports the Clinical Pharmacy Team. Reporting to the Manager, Clinical Pharmacy, the Specialist manages assigned program activities and patient follow-up, coordinates with providers, pharmacy teams, facilities, and external partners, and works to remove non-clinical barriers that affect medication access, adherence, and completion of clinical pharmacy services. The Specialist develops detailed knowledge of assigned Enhanced Services Programs, Medication Therapy Management support workflows, manufacturer-sponsored services, and other clinical initiatives. The role applies established program requirements to individual cases, owns follow-through from referral through documented completion, and escalates clinical decisions, patient safety concerns, and scope exceptions to a pharmacist or other appropriately licensed clinician. Responsibilities Assigned Program and Caseload Execution Manage an assigned caseload or portfolio of clinical pharmacy program activities from referral or identification through outreach, service coordination, follow-up, documentation, and closeout. Review assigned cases for required information, program eligibility elements, open actions, and service deadlines; develop and maintain a clear follow-up plan within established workflows. Complete patient outreach and program education using approved materials; confirm understanding of next steps and document patient preferences, barriers, and agreed actions. Coordinate required activities with pharmacists, provider offices, facilities, pharmacy locations, caregivers, manufacturers, vendors, or payer partners and verify that handoffs are completed. Prioritize time-sensitive cases, unsuccessful outreach, access barriers, and incomplete service requirements and escalate based on defined criteria. Patient Access, Adherence, and Barrier Resolution Build trusting, professional relationships with patients and caregivers through consistent, respectful, and trauma-informed communication. Identify non-clinical barriers that may affect access, adherence, or service completion, such as communication gaps, scheduling challenges, missing documentation, coverage coordination, or disconnected handoffs. Partner with the appropriate internal or external team to resolve identified barriers and continue follow-up until the issue is resolved, transferred, or escalated. Reinforce pharmacist or provider instructions using approved language and refer all medication, symptom, therapy, and clinical judgment questions to the appropriate licensed clinician. Document outreach attempts, patient-reported barriers, actions taken, responsible parties, outcomes, and next steps in the designated system. Provider, Facility, and Partner Coordination Serve as an operational liaison for assigned programs among patients, providers, healthcare facilities, pharmacy teams, drug manufacturers, vendors, and other program partners. Request and organize required non-clinical records, forms, signatures, confirmations, and program documentation; follow up on incomplete items and escalate delays. Communicate case status, open actions, and service requirements to internal and external partners while maintaining privacy and minimum-necessary information standards. Prepare cases and supporting information for pharmacist review, clearly identifying the question, available history, prior actions, and required decision. Support team huddles and case reviews by presenting concise case status, barriers, actions taken, and decisions needed. Program Quality, Compliance, and Improvement Develop and maintain working knowledge of assigned program manuals, service requirements, documentation standards, and performance expectations. Complete required data capture and reporting accurately and on time; reconcile assigned records and correct or escalate discrepancies before submission. Conduct program-level quality checks on assigned work for completeness, timeliness, required documentation, and appropriate escalation. Monitor work against established goals and alert the Manager, Clinical Pharmacy to trends, risks, capacity constraints, or requirements at risk of being missed. Identify recurring patient barriers, partner handoff issues, workflow gaps, and documentation challenges and bring specific examples to the Manager, Clinical Pharmacy. Participate in testing and implementation of approved workflows, job aids, templates, scripts, and tracking tools; share program knowledge and effective practices with Coordinators and other team members. Required Skills Demonstrated ability to manage a caseload, organize complex follow-up, and meet competing program deadlines. Strong patient engagement, service recovery, problem-solving, and cross-functional coordination skills. Effective verbal and written communication, including concise case summaries and clear documentation. Ability to learn detailed program requirements, apply defined criteria consistently, and recognize when escalation is required. Proficiency with Microsoft Office and ability to learn pharmacy, clinical program, and documentation systems. Knowledge of pharmacy or medical terminology and understanding of the behavioral health community. Ability to protect confidential information and communicate with empathy across diverse populations. Education and Experience High school diploma or equivalent required; additional healthcare, pharmacy, or community health education preferred. 3+ years of experience supporting patients in a behavioral health, pharmacy, clinical program, care coordination, or comparable healthcare setting required. Previous behavioral health pharmacy experience preferred. Experience coordinating manufacturer, payer, Enhanced Services, MTM, specialty pharmacy, or other structured patient programs preferred. Pharmacy Technician registration or certification may be required when assigned duties or applicable regulations require it. Working Conditions Full-time, typically requiring 40 hours per week. Work is performed in an office and remote-work setting, based on business needs. Prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.
