Qualifications Certified Phlebotomist, Certified Medical Assistant, Certified Nursing Assistant, Initial Phlebotomy Certification or transcript demonstrating Phlebotomy program completion resulting in successful certification, AND High School diploma, GED, or equivalent certificate.
