In this role, you will deliver personalized physical therapy services directly in patients' homes, promoting recovery, improving mobility, and enhancing quality of life. You will assess patient needs, develop tailored treatment plans, and collaborate with a multidisciplinary team to ensure comprehensive care. Responsibilities Conduct thorough patient evaluations to determine physical therapy needs, focusing on mobility, strength, balance, and pain management. Develop and implement individualized care plans aligned with clinical best practices and patient goals. Deliver hands-on therapy interventions such as therapeutic exercises, manual therapy techniques including trigger point therapy, and modalities tailored to each patient's condition. Monitor patient progress regularly, adjusting treatment strategies as necessary to optimize outcomes. Educate patients and their families on injury prevention, home exercise programs, and self-care techniques to support ongoing recovery. Coordinate discharge planning and communicate effectively with healthcare professionals to ensure seamless transitions of care. Maintain accurate, timely documentation in electronic health records (EHR) systems in compliance with clinical documentation standards and medical terminology requirements. Skills Proven experience working with geriatric patients in home health or outpatient rehabilitation settings. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and geriatrics to inform treatment plans. Expertise in outpatient clinic experience or inpatient hospital environments is highly desirable. Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems) management. Ability to collaborate effectively with healthcare professionals across disciplines for comprehensive patient care. Skilled in discharge planning, treatment plan development, and patient rehabilitation strategies. Excellent client communication skills to educate and motivate patients throughout their recovery journey.
