Taneja Physical Therapy & Wellness is seeking licensed Physical Therapists to provide one-on-one physical therapy in patients' homes throughout Weston, CT and nearby areas. You decide whether the case fits your location and availability. Once accepted, you coordinate visit times with the patient and manage your schedule. Responsibilities Evaluate and treat patients in their homes Develop individualized plans of care Provide therapeutic exercise, gait and balance training, functional training, hands-on treatment, and patient education as appropriate Complete timely and compliant clinical documentation Communicate with our care coordinator, physicians, and other healthcare providers as needed Requirements Active Connecticut State Physical Therapist license Graduate of an accredited physical therapy program Reliable transportation Comfortable providing care independently in patients' homes Strong communication and documentation skills Current CPR certification New graduates are encouraged to apply.
