Apply now to join our team and be part of something meaningful. Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating patients' physical conditions, developing personalized treatment plans, and implementing therapeutic interventions to improve mobility and quality of life. This role may involve working in various settings, including home health, clinics, acute care hospitals, and inpatient facilities. Duties Assess patients' physical abilities and limitations through comprehensive evaluations. Develop individualized care plans that address specific patient needs and goals. Implement therapeutic exercises and interventions to enhance mobility and strength. Collaborate with other healthcare professionals to ensure coordinated patient care. Educate patients on treatment plans, exercises, and rehabilitation techniques. Monitor patient progress and adjust treatment plans as necessary. Participate in discharge planning to ensure continuity of care post-treatment. Maintain accurate documentation of patient evaluations, treatments, and progress notes. Skills Proficient knowledge of physiology and anatomy relevant to physical therapy practices. Experience in Pelvic Health settings is preferred but not required- training would be required. Experience and proficiency in outpatient orthopedics. Familiarity with developing care plans tailored to individual patient needs. Understanding of orthopedic conditions and rehabilitation techniques. Strong communication skills for effective interaction with patients, families, and healthcare teams.
