13 weeks $65.00/hr 8-430p or 830-5p or 9-530p or 930-6p (pending census) 2 weekend day expectations in a month's period.

+ Performs evaluations and re-evaluations to assess patient conditions and progress.

+ Establishes individualized treatment plans based on evaluation results and patient goals.

+ Discharges patients by completing necessary documentation and providing follow-up recommendations.

+ Documents patient care activities accurately and timely in the medical record.

+ Charges for services rendered according to established billing procedures.

+ Educates patients and caregivers about treatment plans, exercises, and home care instructions.

+ Participates in practice changes resulting from performance improvement activities.

+ Supports quality standards and initiatives set by the department.

+ Maintains clinical competencies through ongoing education and training.

+ Meets productivity standards defined by the department.

+ Adapts to changes in patient census by adjusting work hours and assisting in other centers or programs as needed.

+ Performs other duties as assigned.

• Knowledge, Skills, and Abilities:
• + Entry level professional competency of Physical Therapy practices, theory, principles and applications as defined in competencies departmental policy [Required] + Basic computer skills to include Outlook, Microsoft Office and Windows. [Preferred]
