Heartify In Home Care LLC is seeking a dependable and compassionate Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide in-home therapy services for an adult client. The client requires physical therapy three times per week . Visit days and times are flexible and may be arranged directly with the client and family. Responsibilities Provide in-home physical therapy three times weekly Follow the established physician's order and physical therapy plan of care Provide therapeutic exercises, strengthening, balance, mobility, transfer, and gait training as appropriate Help improve the client's safety, endurance, range of motion, and functional independence Educate the client and caregiver on prescribed exercises and safety precautions Monitor progress and report significant changes or concerns Complete accurate visit notes promptly Communicate with Heartify's clinical and administrative team PTA must work under the direction and supervision of the evaluating PT Requirements Active Georgia PT or PTA license Current CPR certification Current TB screening Valid driver's license and reliable transportation Home-health or rehabilitation experience preferred Dependable, professional, and able to manage documentation promptly
