Job Description

Home Health Physical Therapist (PT) - Role Description The Home Health Physical Therapist provides skilled rehabilitative care focused on restoring mobility, improving functional independence, and preventing further physical decline. This role requires a clinician capable of developing individualized therapy plans, implementing evidence-based interventions, and ensuring safe recovery within the patient's home environment.
Core Responsibilities Functional Mobility Training:
Provide gait training, transfer training (bed, chair, toilet), and balance exercises to improve safe ambulation and reduce fall risk.
Therapeutic Exercise Implementation:
Design and administer individualized exercise programs targeting strength, endurance, flexibility, and coordination.
Pain Management & Recovery Support:
Utilize therapeutic modalities and manual techniques to reduce pain, improve circulation, and promote healing.
Fall Prevention & Safety Assessment:
Conduct comprehensive home safety evaluations and recommend modifications (assistive devices, environmental adjustments) to prevent injury.
Care Plan Development & Progression:
Establish measurable goals based on patient condition and continuously adjust treatment plans according to progress and tolerance.
Clinical Assessment:
Perform initial and ongoing evaluations of musculoskeletal, neurological, and functional status, including range of motion (ROM), strength, and mobility limitations.
Documentation:
Maintain accurate and detailed therapy notes, including objective measurements, patient progress, response to treatment, and goal attainment.
Mandatory Qualifications Licensure:
Current, unrestricted Physical Therapist (PT) license.
Clinical Proficiency:
Experience in home health, orthopedic, neurological, or post-acute rehabilitation preferred.
Certifications:
Active BLS/CPR certification.
Health Screening:
Proof of a negative TB (Tuberculosis) test within the last year.
Transportation:
Reliable vehicle, valid driver's license, and active auto insurance for home visits.
Key Skills Rehabilitation Expertise:
Strong knowledge of therapeutic interventions, mobility training, and recovery protocols.
Clinical Judgment:
Ability to assess functional deficits and modify treatment plans based on patient response.
Patient Education:
Ability to instruct patients and caregivers on home exercise programs, proper body mechanics, and fall prevention strategies.
Communication:
Effectively collaborate with physicians, nurses, and interdisciplinary team members.
Motivation & Engagement:
Encourage patient participation and adherence to therapy plans to maximize outcomes.
Reliability:
Consistent adherence to scheduled visits and documentation timelines.
Pay:
$40.00 - $70.00 per hour
