Responsibilities Perform comprehensive patient assessments in the home setting Develop and implement individualized treatment plans focused on functional goals Provide therapeutic exercises, manual therapy, and mobility training Educate patients and caregivers on treatment plans, safety, and home exercises Collaborate with physicians, nurses, and interdisciplinary care teams Monitor patient progress and adjust care plans as needed Maintain accurate and timely clinical documentation Ensure compliance with home health regulations and agency standards
Qualifications Education:
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Experience:
One year of prior experience caring for similar patient population as to be assigned
Certificate/License:
Licensed as a Physical Therapist in the state of Iowa. You will be required to obtain a WI and IL license within 12 months of employment Must hold a valid driver license and automobile insurance
